Based within Tagsa Uibhist’s Care Department, this role is also an integral part of the NHS Western Isles Community Mental Health Team , being clinically supervised within this team and work ing within NHS referral systems.
This role also leads on the coordination of Tagsa Uibhist’s well established Elderly and Dementia Support Service. The Post Diagnosis Support Coordinator / Link Worker (Dementia) is a position open to both health and social work /care staff. The core purpose of the post is to ensure that a minimum of one year’s post diagnostic support is provided to those people newly diagnosed with dementia.
This involves working directly with patients and their families, as well as line managing Tagsa Uibhist Support Workers to provide this service. The PDS Coordinator / Link Worker will enhance the support already available from the NHS WI Community Mental Health Team.
Support will be provided in line with the 5 Pillar Model for people who have received a diagnosis of dementia and will work with families for a minimum of a year, providing advice and support around aspects which will enable people to live well with dementia for as long as possible.
Tagsa Uibhist’s Elderly and Dementia Support Service is funded by Comhairle nan Eilean Siar as well as private donors, trust and foundations to provide a broad range of life – enhancing support and activities for people living with dementia, from home visits, social and cultural activities, outings and healthy activities, for example in our Community Gardens. The EDS service also provides respite care to clients’ families and carers.
